ABSTRACT

Abstract Objectives Evidence suggests that ketamine's influence on brain-derived neurotrophic factor (BDNF) might be involved in its mechanism of rapid antidepressant action. We aimed to evaluate the differential impact of ketamine and esketamine on serum BDNF levels and its association with response patterns in treatment-resistant depression (TRD). Methods Participants (n = 53) are from a randomized, double-blind clinical trial comparing the efficacy of single-dose ketamine (0.5mg/kg, n = 27) and esketamine (0.25mg/kg, n = 26) in TRD. Depression severity was assessed before and 24 hours, 72 hours, and 7 days after the intervention, using the Montgomery-Åsberg Depression Rating Scale (MADRS). Blood samples were collected before infusion, 24 hours, and 7 days afterwards. Results There were no significant changes in BDNF levels at post-infusion evaluation points, and no difference in BDNF levels comparing ketamine and esketamine. Both drugs exhibited similar therapeutic effect. There was no association between BDNF levels and response to treatment or severity of depressive symptoms. Conclusion There was no significant treatment impact on BDNF serum levels - neither with ketamine nor esketamine - despite therapeutic response. These results suggest that ketamine or esketamine intervention for TRD has no impact on BDNF levels measured at 24 hours and 7 days after the infusion. ABSTRACT

To evaluate clinical predictors of poor sleep quality in COPD patients with and without obstructive sleep apnea (OSA). Methods: Consecutive stable patients with COPD were evaluated for OSA by means of overnight polysomnography; for sleep quality by means of the Pittsburgh Sleep Quality Index (PSQI); and for disease impact by means of the COPD Assessment Test. COPD severity was graded in accordance with the 2020 GOLD guidelines. Predictors of poor sleep quality were evaluated by multivariate logistic regression analysis. Results: We studied 51 patients with COPD alone and 51 patients with COPD and OSA. Both groups had similar age (66.2 ± 9.2 years vs. 69.6 ± 10.7, p = 0.09) and airflow limitation (p = 0.37). Poor sleep quality was present in 74.8% of the study participants, with no significant difference between COPD patients with and without OSA regarding PSQI scores (p = 0.73). Polysomnography showed increased stage 1 non-rapid eye movement sleep and arousal index, as well as reduced sleep efficiency and stage 3 non-rapid eye movement sleep, in the group of patients with COPD and OSA (p < 0.05). Independent predictors of poor sleep quality were GOLD grade C/D COPD (OR = 6.4; 95% CI, 1.79-23.3; p < 0.01), a COPD Assessment Test score ≥ 10 (OR = 12.3; 95% CI, 4.1-36.5; p < 0.01), and lowest SaO2 < 80% (p < 0.0001). Conclusions: Poor sleep quality is quite common in patients with COPD and is associated with severe COPD and poor health status, having a negative impact on overall quality of life. Despite changes in polysomnography, OSA appears to have no impact on subjective sleep quality in COPD patients.


Avaliar os preditores clínicos de má qualidade do sono em pacientes com DPOC, com e sem apneia obstrutiva do sono (AOS). Métodos: Pacientes estáveis consecutivos com DPOC foram avaliados quanto à AOS por meio de polissonografia noturna; quanto à qualidade do sono por meio do Índice de Qualidade do Sono de Pittsburgh (IQSP) e quanto ao impacto da doença por meio do Teste de Avaliação da DPOC. A gravidade da DPOC foi classificada conforme as diretrizes de 2020 da GOLD. Os preditores de má qualidade do sono foram avaliados por meio de análise de regressão logística multivariada. Resultados: Foram estudados 51 pacientes com DPOC apenas e 51 pacientes com DPOC e AOS. Ambos os grupos eram semelhantes quanto à idade (66,2 ± 9,2 anos vs. 69,6 ± 10,7, p = 0,09) e limitação do fluxo aéreo (p = 0,37). Sono de má qualidade esteve presente em 74,8% dos participantes, sem diferença significativa entre os pacientes com DPOC, com e sem AOS, quanto à pontuação no IQSP (p = 0,73). A polissonografia mostrou aumento do estágio 1 do sono non-rapid eye movement e do índice de despertares, bem como redução da eficiência do sono e do estágio 3 do sono non-rapid eye movement nos pacientes com DPOC e AOS (p < 0,05). Os preditores independentes de má qualidade do sono foram DPOC grau C/D da GOLD (OR = 6,4; IC95%: 1,79-23,3; p < 0,01), pontuação ≥ 10 no Teste de Avaliação da DPOC (OR = 12,3; IC95%: 4,1-36,5; p < 0,01) e menor SaO2 < 80% (p < 0,0001). Conclusões: O sono de má qualidade é bastante comum em pacientes com DPOC e apresenta relação com DPOC grave e estado de saúde ruim, além de ter impacto negativo na qualidade de vida global. Não obstante as alterações na polissonografia, a AOS aparentemente não tem impacto na qualidade do sono referida por pacientes com DPOC.

RESUMEN Introducción: La enfermedad valvular constituye una patología prevalente y con considerable morbimortalidad. A pesar de que representa una problemática de salud pública reconocida, en nuestro país, no disponemos de datos sobre su epidemiología. El objetivo del trabajo fue identificar la prevalencia y etiología de las enfermedades valvulares en los pacientes que concurren a los laboratorios de ecocardiografía en forma ambulatoria. Materiales y métodos: Se realizó un registro retrospectivo, observacional y multicéntrico, en el que participaron 18 centros de Argentina. A todos los pacientes se les realizó un ecocardiograma transtorácico completo, en el cual el análisis de la función valvular fue realizado por operadores calificados. Se recolectaron variables demográficas, causas y grado de compromiso de la función valvular. Resultados: Se incluyeron 3020 pacientes (51,2% de sexo masculino), con una edad media de 58,2 ± 16,8 años. En el 61,7%, se observó algún grado de alteración valvular; en el 9,5%, al menos, una valvulopatía de grado significativo (moderado o mayor) y en el 1,7% valvulopatías significativas combinadas. El 4% de la población presentó estenosis aórtica significativa, que fue la valvulopatía significativa más frecuente, seguida por la insuficiencia mitral (2,9%) y la insuficiencia tricuspídea (2,3%). La etiología degenerativa fue la causa más frecuente de disfunción valvular aórtica y mitral, y la causa funcional la principal de insuficiencia tricuspídea significativa. Las valvulopatías significativas aumentaron significativamente con la edad, y afectaron al 23,5% de los mayores de 75 años. La estenosis mitral y la insuficiencia tricuspídea fueron más frecuentes en pacientes de sexo femenino, sin encontrarse diferencias de género en las valvulopatías aórticas y la insuficiencia mitral. Conclusiones: Las enfermedades valvulares fueron un hallazgo frecuente en nuestra población. Su prevalencia aumentó con la edad; la etiología degenerativa fue la principal causa de compromiso valvular aórtico y mitral, y la funcional de la insuficiencia tricuspídea. La estenosis mitral y la insuficiencia tricuspídea tuvieron diferencias de acuerdo al sexo. Nuestro trabajo aporta información importante y novedosa sobre la epidemiología de las valvulopatías en Argentina.


ABSTRACT Background: Valvular heart disease is a prevalent pathology with considerable morbidity and mortality. Despite it represents a well-known public health problem, there is no epidemiological data available in our country. The aim of this study was to assess the prevalence and etiology of valvular heart disease in ambulatory patients referred for transthoracic echocardiography. Methods: A retrospective, observational and multicenter registry was carried out with the participation of 18 Argentine centers. All patients underwent a complete transthoracic echocardiogram, in which the analysis of valvular function was performed by qualified operators. Demographic variables, causes and degree of valvular function involvement were collected. Results: A total of 3020 patients (51.2% male) with mean age of 58.2 ± 16.8 years were included in the study. In 61.7% of patients some degree of valvular abnormality was observed; in 9.5% of cases at least one significant valve disease (moderate or greater) and in 1.7% a significant combined valve disease was observed. Four percent of the population had significant aortic stenosis, which was the most common major valve disease, followed by mitral regurgitation (2.9%) and tricuspid regurgitation (2.3%). The most frequent cause of aortic and mitral valve defect was degenerative and functional for significant tricuspid regurgitation. Significant heart valve disease increased notably with age, affecting 23.5% of patients older than 75 years. Mitral stenosis and tricuspid regurgitation were more frequent in female patients, with no gender differences in aortic valve disease and mitral regurgitation. Conclusions: Valvular heart diseases were a frequent finding in our population and their prevalence increased with age. Degenerative etiologies were the main cause of aortic and mitral valve involvement, while a functional etiology was responsible for most cases of tricuspid regurgitation. Gender-specific patterns were observed for mitral stenosis and tricuspid regurgitation. Our findings provide relevant unique data on the epidemiology of heart valve disease in Argentina.

RESUMEN Introducción: Una proporción significativa de los pacientes con válvula aórtica bicúspide (VAB) desarrollan una dilatación de la aorta que los predispone a serias complicaciones. Objetivos: Estimar la prevalencia de dilatación aórtica aplicando los valores de referencia de la población argentina en pacientes con VAB y la influencia de los distintos métodos de indexación (talla, T, y superficie corporal, SC). Materiales y métodos: Se incluyeron consecutivamente 581 pacientes adultos con VAB. Se definió la dilatación según el criterio propuesto por las guías (fórmulas de Devereux) y sobre la base de los valores propuestos por el registro MATEAR (Medición de Aorta Torácica por Ecocardiografía en Argentina). Resultados: La edad media fue de 44,9 años (±16), 68,7% sexo masculino. Sobre la base del registro MATEAR se observó alta prevalencia de dilatación de la raíz aórtica o aorta ascendente (72,3% según T y 61,5% según SC) que resultó, en la raíz, mayor que la obtenida según las fórmulas de Devereux (T 47% vs. 31,5%; SC 35,2% vs. 26,5% p < 0,001). Se observó una subestimación sistemática al indexar por SC en pacientes con índice de masa corporal >25 kg/m² (57,8% de la población). Conclusiones: La prevalencia de dilatación aórtica, cuando aplicamos los valores de referencia para la población argentina, fue alta y en la raiz significativamente mayor que la determinada por puntos de corte originados en otras poblaciones. Se observó una subestimación sistemática al corregir por superficie corporal en pacientes con índice de masa corporal >25 kg/m², por lo que indexar por talla sería la opción más recomendable.


ABSTRACT Background: A significant proportion of patients with bicuspid aortic valve (BAV) develop aortic dilation predisposing to serious complications. Objective: The aim of this study was to estimate the prevalence of aortic dilation applying reference values for the Argentine population in patients with BAV, and the influence of different indexing methods [height, (H) and body surface area (BSA)] Methods: A total of 581 adult patients with BAV were consecutively included in the study. Aortic dilation was defined according to guideline criteria (Devereux formula) and the reference values suggested by the Echocardiography Thoracic Aortic Assessment in Argentina (MATEAR) registry. Results: Mean age was 44.9±16 years and 68.7% were men. A high prevalence of aortic root or ascending aorta dilation was observed based on MATEAR criteria (72.3% according to H and 61.5 % according to BSA). This was significantly higher for the aortic root than the one obtained with the Devereux formula (H: 47% vs. 31.5%; BSA: 35.2% vs. 26.5% P <0.001). A systematic underestimation was found when indexing for BSA in patients with body mass index (BMI) >25 kg/m² (57.8% of population). Conclusions: When applying the reference values for the Argentine population the prevalence of aortic dilation was high and significantly greater than at the root that determined by cutoff points originating in other populations. Systematic underestimation was observed when correcting for BSA in patients with BMI >25 kg/m², so indexing by H would be the most recommended option.

ABSTRACT

OBJECTIVES: To describe the MORPHEOS (Morbidity in patients with uncontrolled HTN and OSA) trial, and describe the challenges imposed by the COVID-19 pandemic. METHODS: MORPHEOS is a multicenter (n=6) randomized controlled trial designed to evaluate the blood pressure (BP) lowering effects of treatment with continuous positive airway pressure (CPAP) or placebo (nasal strips) for 6 months in adult patients with uncontrolled hypertension (HTN) and moderate-to-severe obstructive sleep apnea (OSA). Patients using at least one antihypertensive medication were included. Uncontrolled HTN was confirmed by at least one abnormal parameter in the 24-hour ABPM and ≥80% medication adherence evaluated by pill counting after the run-in period. OSA was defined by an apnea-hypopnea index ≥15 events/hours. The co-primary endpoints are brachial BP (office and ambulatory BP monitoring, ABPM) and central BP. Secondary outcomes include hypertension-mediated organ damage (HMOD) to heart, aorta, eye, and kidney. We pre-specified several sub-studies from this investigation. Visits occur once a week in the first month and once a month thereafter. The programmed sample size was 176 patients but the pandemic prevented this final target. A post-hoc power analysis will be calculated from the final sample. ClinicalTrials.gov: NCT02270658. RESULTS: The first 100 patients are predominantly males (n=69), age: 52±10 years, body mass index: 32.7±3.9 kg/m2 with frequent co-morbidities. CONCLUSIONS: The MORPHEOS trial has a unique study design including a run-in period; pill counting, and detailed analysis of hypertension-mediated organ damage in patients with uncontrolled HTN that will allow clarification of the impact of OSA treatment with CPAP.

ABSTRACT

Calf diarrhea causes substantial economic losses in the cattle industry worldwide. Bovine rotavirus A (RVA) is the main viral agent that leads to enteric infection and diarrhea outbreaks in calves throughout the world. The aim of this retrospective (2006-2015) study was to determine the frequency of RVA detection in diarrheic fecal samples from beef and dairy calves from the three main cattle-producing regions of Brazil. Diarrheic fecal samples (n=1,498) of 124 beef and 56 dairy cattle herds from the Midwest, South, and Southeast geographical regions of Brazil were evaluated using the silver-stained polyacrylamide gel electrophoresis (ss-PAGE) technique. RVA double stranded-RNA was identified by the ss-PAGE technique in 410 (27.4%) fecal samples. The frequency of positive samples found in beef calves (31.9%; 328/1,027) was higher than the frequency found in diarrheic fecal samples from dairy calves (17.4%; 82/471). RVA infection was identified in calves from the three Brazilian geographical regions analyzed. However, the frequency of positive diarrheic calves in the Midwest region (39.4%), predominantly beef calves, was higher than in the South (19.4%) and Southeast (17.6%) regions. The temporal distribution of RVA-infected calves evaluated by two five-year periods (2006-2010, 24.5%; 2011-2015, 28.8%) demonstrated a very similar frequency of RVA in both periods. Considering the wide regional and temporal scope of this study, it can be concluded that RVA remains an important etiology of neonatal diarrhea in calves of Brazilian cattle herds.(AU)


A diarreia neonatal ocasiona perdas econômicas importantes na pecuária bovina em todo o mundo. Rotavírus A (RVA) é o principal agente etiológico viral de infecções entéricas e surtos de diarreia em bezerros de rebanhos de corte e leite. O objetivo deste estudo retrospectivo (2006-2015) foi determinar a frequência de detecção de RVA em amostras de fezes diarreicas de bezerros de corte e leite das três principais regiões produtoras de bovinos do Brasil. Amostras de fezes diarreicas (n=1.498) de 124 rebanhos bovinos de corte e 56 rebanhos bovinos de leite das regiões Centro-Oeste, Sul e Sudeste do Brasil foram avaliadas utilizando a técnica de eletroforese em gel de poliacrilamida (EGPA). O genoma segmentado de RVA foi identificado pela técnica de EGPA em 410 (27,4%) amostras de fezes. A frequência de amostras positivas encontrada em bezerros de rebanhos de corte (31,9%; 328/1.027) foi maior que a frequência identificada em amostras de fezes diarreicas de bezerros de rebanhos leiteiros (17,4%; 82/471). A infecção por RVA foi identificada em bezerros das três regiões geográficas brasileiras analisadas. No entanto, a frequência de bezerros com diarreia positivos para RVA na região Centro-Oeste (39,4%), predominantemente de bezerros de rebanhos de corte, foi maior que nas regiões Sul (19,4%) e Sudeste (17,6%). A distribuição temporal dos bezerros infectados com RVA avaliados por dois períodos de cinco anos (2006-2010, 24,5%; 2011-2015, 28,8%) demonstrou uma frequência muito semelhante em ambos os períodos. Considerando a amplitude regional e temporal deste estudo, pode-se concluir que RVA continua sendo uma importante etiologia de diarreia neonatal em bezerros de rebanhos bovinos brasileiros.(AU)

ABSTRACT

Although Leishmania infantum is well-known as the aethiological agent of visceral leishmaniasis (VL), in some Central American countries it may cause atypical non-ulcerated cutaneous leishmaniasis (NUCL). However, the mechanisms favoring its establishment in the skin are still unknown. Lipophosphoglycan (LPG) is the major Leishmania multivirulence factor involved in parasite-host interaction. In the case of viscerotropic L. infantum, it causes an immunosuppression during the interaction with macrophages. Here, we investigated the biochemical and functional roles of LPGs from four dermotropic L. infantum strains from Honduras during in vitro interaction with murine macrophages. LPGs were extracted, purified and their repeat units analysed. They did not have side chains consisting of Gal(β1,4)Man(α1)-PO4 common to all LPGs. Peritoneal macrophages from BALB/c and C57BL/6 were exposed to LPG for nitric oxide (NO) and cytokine (TNF-α and, IL-6) production. LPGs from dermotropic strains from Honduras triggered higher NO and cytokine levels compared to those from viscerotropic strains. In conclusion, LPGs from dermotropic strains are devoid of side-chains and exhibit high pro-inflammatory activity.

Abstract Objective: To verify the effect of psychological preparation on the relief of preoperative anxiety in children and to correlate parents' and children's levels of anxiety. Method: After the approval of the institutional Research Ethics Committee and written consent of the children's parents or guardians, 118 children of both genders were prospectively selected, aged between 2 and 8 years, physical condition classification ASA I, who were treated in the pre-anesthetic evaluation ambulatory of the University Hospital and who underwent ambulatory surgeries at the same hospital. Two controlled groups of 59 children were randomized: control group basic preparation and psychological preparation group. On the day of surgery, all selected children were evaluated regarding their level of anxiety using the modified Yale Preoperative Anxiety Scale and their parents were evaluated regarding their level of anxiety through the Visual Analog Scale. The evaluator was blinded to which study group the child and family member belonged to. Results: Nine children and their family members were excluded per group when the results were analyzed. Children from the prepared group showed significant reductions in their level of anxiety in relation to the control group (p = 0.04). There was no correlation between the level of anxiety of children and their parents' levels (p = 0.78). Conclusion: The psychological preparation was effective in reducing the level of anxiety of children. However, there was no relation between the level of anxiety of children and their parents' level.


Resumo: Objetivos: Verificar o efeito da preparação psicológica no alívio da ansiedade pré-operatória de crianças e avaliar se há correlação com a ansiedade dos pais. Método: Após a aprovação do Comitê de Ética e Pesquisa da Faculdade de Medicina e obtenção do consentimento pelos responsáveis dos pacientes, foram selecionadas prospectivamente 118 crianças, de ambos os sexos, entre dois e oito anos, classificação de estado físico ASA I, atendidas no ambulatório de avaliação pré-anestésica do Hospital Universitário e submetidas a cirurgias ambulatoriais. Foram randomizados dois grupos controlados de 59 crianças: grupo de preparação básica e grupo de preparação psicológica. No dia da cirurgia, todas as crianças foram avaliadas em relação ao seu grau de ansiedade através da Escala de Ansiedade Pré-operatória de Yale Modificada e seus pais, avaliados quanto ao seu nível de ansiedade pela Escala Visual Analógica. O avaliador era cego sobre qual grupo do estudo a criança e seu familiar pertenciam. Resultados: Na análise dos resultados, foram excluídas nove crianças e familiares de cada grupo. As crianças do grupo preparado tiveram reduções significativas no grau de ansiedade em relação ao grupo controle, (p = 0,04). Não houve correlação entre os graus de ansiedade das crianças e seus pais (p = 0,78). Conclusão: A preparação psicológica foi eficaz na redução do grau de ansiedade das crianças no momento da cirurgia. Não houve, entretanto, relação entre os graus de ansiedade dos pais e seus filhos.

ABSTRACT

In dogs with congestive heart failure, the upregulated sympathetic tone causes vasoconstriction that impairs peripheral blood supply, therefore causing the accumulation of lactate. In this prospective cross-sectional study with a longitudinal component, blood lactate was quantified in 10 healthy and 34 myxomatous mitral valve disease (MMVD) dogs to investigate its potential use as a diagnostic and prognostic biomarker. While there were no differences in lactate concentration between control animals and stages B1 (3.31±0.62mmol/L) and B2 (3.32±0.46mmol/L) dogs, significant differences were found between healthy (2.50±0.69mmol/L) and both C (3.99±0.47mmol/L) and D (6.97±1.23mmol/L) animals. When a cut-off of 3.35mmol/L was used, lactate was able to distinguish dogs with normal and remodeled hearts with a sensitivity of 78.2% and specificity of 63.6%. Also, significant correlations existed between lactate and indicators of cardiac remodeling. Finally, animals with blood lactate <3.5mmol/L carried a better prognosis when compared with dogs in which lactate was >5.0mmol/L. Our results suggest that the progression of MMVD results in accumulation of lactate within the bloodstream, which is likely attributable to the impaired peripheral tissue perfusion. In MMVD dogs, blood lactate may be used as a surrogate for cardiac remodeling, and an increased concentration is associated with a worse prognosis regarding the time to evolve into congestive heart failure.(AU)


Em cães com insuficiência cardíaca congestiva, o tônus simpático hiperregulado causa vasoconstrição e interfere com o suprimento sanguíneo periférico, resultando no acúmulo de lactato. Neste estudo prospectivo transversal com um componente longitudinal, o lactato sanguíneo foi quantificado em 10 cães saudáveis e 34 cães com doença mixomatosa da valva mitral (DMVM) para investigar seu potencial como biomarcador diagnóstico e prognóstico. Embora não tenham sido identificadas diferenças na concentração de lactato entre animais controle e cães com DMVM nos estágios B1 (3,31±0,62mmol/L) e B2 (3,32±0,46mmol/L), diferenças significativas foram constatadas entre os cães saudáveis (2,50±0,69mmol/L) e cães com DMVM estágio C (3,99±0,47mmol/L) ou D (6,97±1,23mmol/L). Quando utilizado o valor de corte de 3,35mmol/L, o lactato foi capaz de diferenciar cães com corações normais daqueles com corações remodelados com sensibilidade de 78,2% e especificidade de 63,6%. Além disso, correlações significativas foram encontradas entre o lactato e os indicadores de remodelamento cardíaco. Por fim, os animais com lactato sanguíneo <3,5mmol/L tiveram prognóstico melhor comparativamente aos cães com concentrações >5,0mmol/L. Nossos resultados sugerem que a progressão da DMVM resulta no acúmulo de lactato na corrente sanguínea, fato que é provavelmente atribuído à perfusão periférica prejudicada. Em cães com DMVM, o lactato sanguíneo pode ser utilizado como indicador de remodelamento cardíaco, cuja concentração elevada está associada com pior prognóstico relativo ao tempo para evoluir para insuficiência cardíaca congestiva.(AU)

ABSTRACT Introduction: The harmonization of equipment is recommended in clinical laboratory practice aiming for the homogeneity of results when similar or equivalent analyzers are used to perform routine testing. Objectives: To conduct a study of equivalence between the biochemical analyzers Labmax 240® (E1) and Labmax 240 Premium® (E2) through the matching results and the statistical value analysis of dosages. Materials and methods: We evaluated tests with glucose, total cholesterol, triglycerides, uric acid, aspartate transaminase (AST), alanine transaminase (ALT) and lactate dehydrogenase (LDH), all with 40 repeated measurements, performed in both equipments. The Clinical and Laboratory Standards Institute (CLSI) EP09-3A protocol was used to conduct the comparison test between E1 and E2 equipment, with subsequent evaluation of the results for statistical analysis determining the Pearson correlation coefficient (r) and indexes comparison error with EP Evaluator® software. Results: Regarding the values of the Pearson correlation coefficient, all tests showed a strong correlation between equipment with r > 0.989, except for the dosage of LDH (r = 0.982). This dosage failed not because the value of r, but due to the values obtained in the error index being larger than the total errors index allowed. Discussion: Compared to clinical criteria, the results of the analyzers are approximately equal, but this control process must be done continuously in order to prevent and track random errors within the laboratory routine. Conclusion: The process of harmonization of multiple devices that perform the same laboratory parameters is essential for ensuring quality and reliability of laboratory results and should be standardized and included in routine clinical analysis laboratories.


RESUMO Introdução: No laboratório clínico, é recomendável a harmonização de equipamentos que visem à homogeneidade dos resultados, quando analisadores similares ou equivalentes são utilizados para desempenho da rotina de realização dos testes. Objetivos: Realizar um estudo de equivalência entre os analisadores bioquímicos Labmax 240® (E1) e Labmax 240 Premium® (E2). Materiais e métodos: Foram avaliados os testes glicose (GLI), colesterol total (COL), triglicerídeos (TRI), ácido úrico , aspartato aminotransferase (AST), alanina aminotransferase (ALT) e lactato desidrogenase (LDH), todos com 40 dosagens repetidas, realizadas em ambos os equipamentos. O protocolo do Clinical and Laboratory Standards Institute (CLSI) EP09-3A foi utilizado para conduzir o teste de comparação, com posterior avaliação dos resultados pela análise estatística no software EP Evaluator®, com determinação do coeficiente de correlação de Pearson (r) e comparação de índices de erro. Resultados: Em relação aos valores do coeficiente de correlação de Pearson, todos os testes apresentaram forte correlação entre os equipamentos, com r > 0,989, exceto para a dosagem de LDH (r = 0,982), que foi reprovada, não em função do valor de r, mas devido aos valores obtidos em relação ao índice de erro, o qual é maior do que os índices de erro total permitido. Discussão: Diante dos critérios clínicos, os resultados dos analisadores são aproximadamente iguais, porém esse controle do processo deve ser feito continuamente a fim de impedir e rastrear erros aleatórios dentro da rotina laboratorial. Conclusão: O processo de harmonização de múltiplos equipamentos que realizam os mesmos parâmetros laboratoriais é fundamental para a garantia da qualidade e da confiabilidade dos resultados laboratoriais, devendo ser padronizado e incluído na rotina dos laboratórios de análises clínicas.

ABSTRACT Laugier-Hunziker syndrome (LHS) is a rare mucocutaneous disorder, of unknown etiology, characterized by multiple hyperpigmented macules, dispersed mostly on the oral mucosa, occasionally associated with longitudinal ridging of the nails. The diagnosis requires exclusion of other conditions, such as Addison's disease and Peutz-Jeghers syndrome. We report a case of a 34-year-old male patient, presenting with hyperpigmented macules on the lips, buccal mucosa and palate, as well as mild dark striations on toenails. After careful clinical and laboratorial investigations, the diagnosis of LHS was established. Given the lack of aesthetic complaints and symptoms, no treatment was necessary.


RESUMO A síndrome de Laugier-Hunziker (SLH) é uma rara desordem mucocutânea, de etiologia indeterminada, caracterizada por múltiplas máculas hiperpigmentadas, dispersas principalmente na mucosa oral, por vezes associadas a estrias longitudinais nas unhas. O diagnóstico requer exclusão de condições como doença de Addison e síndrome de Peutz-Jeghers. Descrevemos o caso de um paciente do sexo masculino, 34 anos, com presença de máculas hiperpigmentadas em lábios, mucosa jugal e palato, além de discretas estrias enegrecidas nas unhas dos pés. Após minuciosa investigação clínica e laboratorial, foi estabelecido o diagnóstico de SLH. Dada a ausência de queixas estéticas e sintomatologia, nenhum tratamento foi necessário.

ABSTRACT

O ballet clássico é uma forma de dança tradicional que utiliza a ponta como posição básica, com o pé em posições extremas, predispondo-o à diversas alterações morfológicas. O objetivo deste estudo foi analisar as impressões plantares de praticantes de ballet clássico, e correlacionar o tipo de pé com o tempo de prática de ballet e o tempo de uso de sapatilha de ponta. A amostra foi composta por 34 praticantes de ballet, do gênero feminino, de escolas de dança no município de Mineiros e Goiânia em Goiás. As variáveis registradas foram sexo, idade, tempo de prática de ballet e tempo de uso de sapatilha de ponta, medida do Índice de Massa Corporal (IMC) e avaliação da impressão plantar através da plantigrafia e baropodometria. A média de idade das praticantes foi de 18,059,39 anos, média do IMC de 18,79 kg/m2, 10,457,68 anos em média de tempo de prática de ballet, e média de 5,365,69 anos do tempo de uso de sapatilha de ponta. Análise geral das 68 impressões plantares apontou predomínio de pés planos. Quando a análise foi feita individualmente por pés, direito e esquerdo, evidenciou predomínio de pés planos e cavos, respectivamente. Os achados baropodométricos demonstraram maior pressão e área plantar nos pés do lado direito (quadrante anterior) e distribuição de pressão semelhante em ambos os pés. Não foi observada correlação significativa entre os tipos de pés e o tempo de prática de ballet clássico, e o tempo de uso de sapatilha de ponta. Classic ballet is a traditional form of dance that uses the pointe as the basic position, with the foot in extreme positions, predisposing it to morphological changes. The objective of this study were to analyze the footprints of classic ballet practicers, and correlate the foot type with the practice time of ballet and time of use of pointe shoe. The sample consisted of 34 ballet dancers coming from dance schools in the cities of Goiania and Mineiros, state of Goias, Brazil. The personal characteristics recorded were sex, age, length of time practicing ballet and the use of pointe shoe, so as the Body Mass Index (BMI) and evaluation of footprints through plantigraphy and baropodometry. The average age of the ballet practicers was 189,39 years, with normative values of body mass, 107,68 years as the time average of ballet practice, and a time average of 55,69 years regarding the use of pointe shoes. An overview of the 68 footprints pointed prevalence of flat feet. Individual analysis of the feet, right and left, revealed a predominance of flat feet and cavus, respectively. The baropodometric findings demonstrated greater pressure on the feet and plantar area of the right side (anterior quadrant) and similar distribution of pressure on both feet. No significant correlation was observed between the types of feet and practice time of classic ballet, neither with the time of use of the pointe shoe. ABSTRACT

Meta-analytical studies have indicated that isometric handgrip training promotes significant reduction in blood pressure in hypertensive patients with similar or greater decreases in blood pressure than observed after aerobic and dynamic resistance training. However, several gaps in the literature still need to be addressed. Thus, we designed the ISOPRESS network group, which consists of a task force of different research groups aimed at analyzing the effects of isometric handgrip training on different contexts, parameters, and populations. Thus, the aim of this study was to describe the rationale and design behind the ISOPRESS, presenting the methods employed. The ISOPRESS questions involve whether isometric handgrip training is effective in hypertensives in different settings (ISOPRESS 1 - unsupervised training and ISOPRESS 2 - public health system), whether it works in patients with other cardiovascular diseases (ISOPRESS 3 - obstructive sleep apnea and ISOPRESS 4 - peripheral artery disease) and what are the mechanisms underlying the effects of isometric handgrip training in hypertensives (ISOPRESS 5 - neural mechanism). The study will yield information on the effectiveness of isometric handgrip training in different settings and patients with other cardiovascular diseases. Finally, it will help to understand the mechanisms involved in reducing blood pressure in hypertensives.(AU)

ABSTRACT Background Recent studies have shown that the magnitude of the metabolic and autonomic responses during progressive resistance exercise (PRE) is associated with the determination of the anaerobic threshold (AT). AT is an important parameter to determine intensity in dynamic exercise. Objectives To investigate the metabolic and cardiac autonomic responses during dynamic resistance exercise in patients with Coronary Artery Disease (CAD). Method Twenty men (age = 63±7 years) with CAD [Left Ventricular Ejection Fraction (LVEF) = 60±10%] underwent a PRE protocol on a leg press until maximal exertion. The protocol began at 10% of One Repetition Maximum Test (1-RM), with subsequent increases of 10% until maximal exhaustion. Heart Rate Variability (HRV) indices from Poincaré plots (SD1, SD2, SD1/SD2) and time domain (rMSSD and RMSM), and blood lactate were determined at rest and during PRE. Results Significant alterations in HRV and blood lactate were observed starting at 30% of 1-RM (p<0.05). Bland-Altman plots revealed a consistent agreement between blood lactate threshold (LT) and rMSSD threshold (rMSSDT) and between LT and SD1 threshold (SD1T). Relative values of 1-RM in all LT, rMSSDT and SD1T did not differ (29%±5 vs 28%±5 vs 29%±5 Kg, respectively). Conclusion HRV during PRE could be a feasible noninvasive method of determining AT in CAD patients to plan intensities during cardiac rehabilitation.

ABSTRACT

The left atrial volume (LAV) can be obtained using the biplane Simpson's method via echocardiography. Although in medicine this parameter has been considered to be a prognostic marker of left atrial enlargement in several cardiac diseases, in dogs with myxomatous mitral valve degeneration (MMVD), a valvulopathy characterized by left atrial (LA) volume overload, atrial enlargement is usually assessed by the LA-to-Aorta ratio (LA:Ao). Therefore, the body surface area (BSA)-indexed LAV was measured in healthy dogs and in dogs with MMVD using the biplane Simpson's method. For this purpose, a total of 107 healthy dogs (control) and 81 dogs with MMVD in ACVIM stages B1, B2 and C were assessed, with LAV being calculated during atrial diastole (d) and systole (s) through the biplane Simpson's method. Two-dimensional apical four-chamber (4C) and two-chamber (2C) images were obtained in every dog through the left parasternal window. The values obtained from healthy dogs were correlated with body weight using Pearson's test. An analysis of variance (ANOVA) and Tukey's test were used to compare healthy and MMVD dogs, as well as to investigate differences according with MMVD stages. A strong positive correlation was documented between either LAVd (r> 0.77) or LAVs (r> 0.73) and body weight in healthy dogs. The BSA-indexed LAV calculated for MMVD dogs was significantly different (p<0.01) from that obtained for the control group. Also, LAV was significantly different (P<0.05) when stages B2 and C, and B1 and C were compared. In conclusion, this study provided a reference for left atrial volume and the applicability of this technique to assess atrial overload in dogs with varying-stage MMVD.(AU)


A mensuração do volume atrial esquerdo (VAE) pode ser obtido pelo método biplanar de Simpson via ecocardiografia. Embora na medicina tal parâmetro venha sendo considerado um marcador prognóstico da sobrecarga atrial esquerda em várias cardiopatias, nos cães com degeneração mixomatosa da válvula mitral (DMVM), valvulopatia caracterizada pela sobrecarga do átrio esquerdo (AE), a avaliação da sobrecarga atrial é realizada rotineiramente apelas pela relação entre o diâmetro do AE e da artéria Aorta (Ao) (AE:Ao). Portanto, procurou-se mensurar os valores do LAV indexados à área de superfície corporal (ASC) em cães sadios e em portadores da DMVM utilizando-se do método biplanar de Simpson. Para tanto, foram avaliados 107 cães sadios (controle) e 81 portadores da DMVM nos estágios B1, B2 e C (ACVIM), sendo o VAE calculado nos momentos da diástole (d) e sístole (s) atrial utilizando o método biplanar de Simpson. Imagens bidimensionais apicais quatro (4C) e duas câmaras (2C) foram obtidas pela janela paraesternal esquerda em todos os cães. Os valores do LAV nos cães sadios foram correlacionados com o peso corporal utilizando o teste de Pearson. Para a comparação dos cães sadios com os portadores da DMVM, bem como para investigar diferenças atribuídas aos diferentes estágios da DMVM, empregou-se análise de variância (ANOVA) e o teste de Tukey. Uma correlação positiva forte foi identificada entre VAEd (r>0,77), VAEs (r>0,73) e o peso corporal dos cães sadios. O LAV indexado pela ASC calculado para os cães portadores de DMVM foi significativamente diferente (p<0,01) daquele obtido no grupo controle. Também foram observadas diferenças significativas (p<0,05) no VAE quando as classes B2 e C, e B1 e C foram comparadas. Em conclusão, este estudo fornece uma referência para o volume atrial esquerdo e a aplicabilidade dessa técnica para avaliação da sobrecarga atrial em cães com DMVM em diferentes estágios.(AU)

ABSTRACT

A mensuração do lactato sérico é utilizada na rotina médica como marcador prognóstico de pacientes em estado de emergência. Sua interpretação não deve ser feita de forma isolada, mas conjunta aos demais parâmetros clínicos, pois seus valores podem sofrer interferência do estresse metabólico ou ambiental, contenção física e/ ou manipulação dos pacientes. Assim, buscou-se mensurar os valores do lactato sérico e parâmetros clínicos de cães saudáveis, bem como as suas correlações, durante o atendimento clínico ambulatorial veterinário. Para isso, foram avaliados 80 cães, machos ou fêmeas, com idade de um a oito anos, atendidos para revacinação anual polivalente. Foram considerados cães saudáveis os que não apresentaram intercorrências clínicas nos últimos 60 dias e alterações nos exames físicos e nos valores de hemograma e glicemia sérica. Foram mensurados inicialmente o peso corporal, a frequência cardíaca (FC) e respiratória (FR), tempo de preenchimento capilar (TPC), coloração de mucosas, temperatura retal (TR), periférica (TP) e a diferença entre TR e a TP, o Delta T°C. Por último, realizaram-se os exames de hemograma e glicemia sérica, juntamente com a mensuração do lactato sérico, utilizando para isso um lactímetro portátil, por meio da amostra sanguínea obtida da veia cefálica. Além disso, havendo a correlação dos valores do lactato séricos com o peso corporal os cães foram divididos conforme o cálculo do 33° e 66° percentil. Os cães avaliados evidenciaram valores médios de 18,3±12,1 kg de peso corporal e 3,0±1,9 anos de idade; FC de 126,6±29,1bpm, FR de 66±24mpm, TR de 38,9±0,4°C, TP de 31,5±1,0°C, Delta TºC de 7,3±1,0°C e lactato sérico de 3,2±0,4mmol/L; com este último, evidenciando intervalo de confiança a 95% de 3,1-3,3mmol/L e correlação significativa (p<0,05) dos seus valores com o peso corporal (r=0,6) e a frequência cardíaca (r=0,4). Os valores do lactato sérico obtidos foram comparados entre os grupos de cães conforme o peso corporal, evidenciando diferenças significativas (p<0,05) entre eles. Dessa forma, pode se concluir que os valores do lactato sérico em cães hígidos sob atendimento ambulatorial é de 3,2mmol/L, com o intervalo de confiança de 3,1-3,3mmol/L, ressaltando a influência que a FC e o peso corporal podem exercer nos seus valores.(AU)


The measurement of serum lactate is used in the medical routine as a prognosis marker of emergency patients. Its interpretation should not be done disconnectedly from the other clinical parameters once metabolical or environmental stress as well as restraint and/or manipulation of patients can interfere. Thus we tried to measure the levels of serum lactate and clinical parameters of healthy dogs, as their correlation during veterinarian outpatient clinical care. For that we evaluated 80 dogs, males and females, with age ranging from one to eight years, met for polyvalent annual revaccination. We considered to be healthy those dogs that had no clinical events in the last 60 days or alteration in physical exams, blood exam values and serum glycemia. We initially measured body weight, heart rate (HR) and respiratory (RR), capillary refill time, mucosa's coloring, rectal temperature (RT), peripheral temperature (PT) and the difference between RT and PT, Delta T°C. Finally we did the blood exam and the serum glycemia, as well as the serum lactate measurement. For that we used a portable lactimeter, using the blood sample taken from the cephalic vein. Furthermore, when there was correlation between the serum lactate values and the body weight, we divided the dogs according to the calculation of 33 and 66 percentile. Evaluated dogs showed average values of 18.3±12.1 kg of body weight and 3.0±1.9 of age; with HR of 126.6±29.1bpm, RR of 66±24mpm, RT of 38.9±0.4°C, PT of 31.5±1,0°C, Delta T°C of 7.3±1.0°C and serum lactate of 3.2±0.4mmol/L; with the latter showing range of 3.1-3.3mmol/L with 95% of reliability and significant correlation (p<0.05) between its values and the body weight (r=0.6) and the heart rate (r=0.4). The serum lactate values obtained were compared between the dogs' groups according to their body weight, showing distinguished differences between them. Thereby we concluded that the serum lactate values in dogs under outpatient care is 3.2mmol/L, with a trust gap of 3.1-3.3mmol/L, highlighting the influence that HR and body weight can have on its values.(AU)

Background: Whole body vibration (WBV) has been used to increase physical activity levels in patients with type 2 diabetes mellitus (T2DM). Objective: To carry out a systematic review of the effects of WBV on the glycemic control, cardiovascular risk factors, and physical and functional capacity of patients with T2DM. Method: MEDLINE, LILACS, PEDro, and Cochrane Central Register of Controlled Trials were searched up to June 1st, 2015. Randomized controlled trials investigating the effects of WBV, compared to control or other intervention, on blood glucose levels, blood and physical cardiovascular risk factors, and physical and functional capacity in adult individuals with T2DM. Two independent reviewers extracted the data regarding authors, year of publication, number of participants, gender, age, WBV parameters and description of intervention, type of comparison, and mean and standard deviation of pre and post assessments. Results: Out of 585 potentially eligible articles, two studies (reported in four manuscripts) were considered eligible. WBV interventions provided a significant reduction of 25.7 ml/dl (95% CI:-45.3 to -6.1; I2: 19%) in 12 hours fasting blood glucose compared with no intervention. Improvements in glycated hemoglobin, cardiovascular risk factors, and physical and functional capacity were found only at 12 weeks after WBV intervention in comparison with no intervention. Conclusion: WBV combined with exercise seems to improve glycemic control slightly in patients with T2DM in an exposure-dependent way. Large and well-designed trials are still needed to establish the efficacy and understand whether the effects were attributed to vibration, exercise, or a combination of both.

Introducción: La miocardiopatía hipertrófica es la miocardiopatía de origen genético más común y en nuestro medio no hay información disponible acerca de las características basales y de la evolución de los pacientes con esta patología. Objetivos: Conocer el perfil clínico de pacientes con miocardiopatía hipertrófica e identificar predictores de mala evolución. Material y métodos: Se incluyeron 143 pacientes con miocardiopatía hipertrófica en el Hospital Italiano de Buenos Aires entre 2005 y 2011. Resultados: La mediana de edad de la población fue de 66 años y el 52% eran mujeres. La mayoría de los pacientes (92%) presentaron distribución asimétrica, el 60% obstrucción dinámica. Con una mediana de seguimiento de 2,11 años (rango intercuartil 25-75: 0,75-3,70), la mortalidad total fue del 5,59%. El síntoma más frecuente fue la disnea (36%); le siguieron la angina (17%) y el síncope (14%). Las variables que se asociaron en forma independiente con la disnea fueron la obstrucción dinámica, la insuficiencia mitral mayor o igual a moderada, el diámetro de la aurícula izquierda y el sexo femenino. La obstrucción dinámica se asoció en forma independiente con la angina. El espesor máximo presentó una relación directa e independiente con el síncope, mientras que la fracción de eyección y la hipertrofia ventricular izquierda o las T negativas en el electrocardiograma tuvieron una relación inversa. Los pacientes que se internaron por insuficiencia cardíaca descompensada tuvieron mayor mortalidad en forma independiente. Conclusiones: Al igual que en otras series, en nuestra población se objetivó que la miocardiopatía hipertrófica es una enfermedad muy heterogénea. Es necesario realizar un estudio prospectivo para validar los predictores de riesgo evaluados en este trabajo.


Introduction: Hypertrophic cardiomyopathy is the most frequent genetic cardiomyopathy and there is no available information on baseline characteristics and outcome of patients with this disease in our country. Objectives: To know the clinical profile of patients with hypertrophic cardiomyopathy and to identify predictors of adverse outcome. Methods: One hundred and forty-three patients with hypertrophic cardiomyopathy at the Hospital Italiano of Buenos Aires between 2005 and 2011 were included in the study. Results: Median age was 66 years and 52% were women. Most patients presented an asymmetric distribution (92%) and 60% had dynamic obstruction. Mortality was 5.59% at a median follow-up of 2.11-years (25-75 IQR: 0.75-3.70). The most prevalent symptom was dyspnea (36%), followed by angina (17%) and syncope (14%). Dynamic obstruction, moderate or severe mitral regurgitation, left atrial diameter and female gender were independently associated with dyspnea. Dynamic obstruction was independently associated with angina. Maximum wall thickness was directly and independently associated with syncope, while ejection fraction and left ventricular hypertrophy or negative T in the electrocardiogram presented an inverse relationship. Mortality was independently associated with hospitalization for decompensated heart failure. Conclusions: Similar to previous studies, our population shows that hypertrophic cardiomyopathy is a heterogeneous disease. A prospective study is necessary to validate the risk predictors assessed in this study.
